Identified as STC 12686a on UMI microfilm reel 597, and as 12686 on reel 1307. Reproductions of the originals in the Folger Shakespeare Library and the Henry E. Huntington Library and Art Gallery. Appears at reel 597 (Folger Shakespeare Library copy) and at reel 1307 (Henry E. Huntington Library and Art Gallery copy).
